As part of a comprehensive care team provide Medical Case Management services to HIV positive adults registered at the EIP. The Standards for Medical Case Management have been developed and approved by the Ryan White grant funder and responsibilities include, but are not limited to, patient assessment, assistance with applications for medications and outside services, initial, annual and six-month certifications for eligibility, patient education, and referrals to outside providers. Accurate documentation must be completed in a timely manner and participation in department-wide QI activities are essential.
Provide grant required Medical Case Management services to 25% of the patients registered at the EIP.
Educational Skills Required for the position (Degree(s), Certification(s), License, etc). Bachelor of Science in Social Services or related fields. Certified Bachelor of Science in Social Services preferred. Ryan White grant required courses CHAMP. Department of Health grant required courses. Department inservice education programs related HIV/AIDS. Case management inservices/seminars/conferences as requested.
