Job Description

This position is for Case Manager on an inpatient medical/surgical unit. This critical position is responsible for many tasks and activities on an inpatient unit, including but not limited to collaboration with the unit medical staff, nursing staff, physician advisors, social worker and other members of the care team to ensure that the patient is appropriate for admission and that all further interventions are both appropriate and timely. In addition, the case manager will do regular clinical reviews of each patient and transmit them to the insurer. The case manager will monitor those cases for approval/denial of days and address such respectively. The case manager will also provide discharge planning for those patients requiring complex home care needs (ie oxygen, infusion therapy) and will coordinate all DC needs on all patients with the care team.

Job Requirements

New Jersey RN license required. National Case Management Certification or pertinent clinical specialty certification preferred. Minimum of 3 years medical-surgical or other clinical experience. Case Management experience preferred.
