Job Description

POSITION SUMMARY:
• Performs interventions for identified elderly patients in the Hospital Acquired Delirium Prevention Program under the direct supervision of the program’s Intervention Clinician (IC) or Staff RN.
• Orients patients and families to the program and perform appropriate intervention protocols of the Orientation Protocol, Therapeutic Activities Protocol, Early Mobilization and Sleep Enhancement Protocol.
POSITION RESPONSIBILITIES INCLUDE:
Program Interventions
1. Implement intervention protocols as assigned by and under the direct supervision of the IC or staff RN.
2. Perform regular evaluations of all patients to track progress, respond to patient care needs, and update care plan.
3. Assisting in the monitoring and recording of all protocols and program outcome variables.
4. Distribute and collect Patient Satisfaction Surveys at the direction of the IC.

Job Requirements

High school graduate required; Bachelor’s Degree Preferred; One to three years working in a related position. Mature person experienced working with elderly individuals preferred. Bilingual Spanish preferred; Enjoy working with elderly; Attention to detail, organizational skills and ability to keep careful documentation; Excellent communication and interpersonal skills; Demonstrated ability to work as part of an interdisciplinary team; Ability to take direction in initiating, prioritizing and managing multiple tasks; Certified Nursing Assistant preferred.
