Job Description

Statewide Clinical Outreach for the Elderly ( S-COPE)

Our Statewide program looks to promote the most efficient use of acute care services and long-term psychiatric services for our clients. In collaboration with the facilities we interface with, the goal is to develop expertise in promoting quality of life, wellness and dignity of each resident. As an S-COPE clinician, the objective is to equip staff of the residential facilities with tools to effectively manage behavioral issues in place.

The role of the MA Clinician is to provide education to screening centers, psychiatric inpatient units, Long Term Care settings on how to access services. Duties include; • Face to face on site assessment and phone consultation • Professional consultation regarding medication management • Recommendations regarding level of support • Treatment and case-specific intervention • Assessment of older adults in order to clarify diagnosis and assist with treatment planning

Job Requirements

Masters degree in Social Work, Counseling or psychology degree. Two years experience working with individuals with gero-psychiatric disabilities. experience in the mental health field.
