Peritoneal Dialysis Nurse

The Peritoneal Dialysis Nurse is responsible for coordinating care and managing operations. Will oversee training, education and home support needed for patients to receive staff assisted dialysis safely in their homes. The Peritoneal Dialysis Nurse works collaboratively with team members, participates in program development and is on-call (if clinical during off hours for patient care and support).
PRIMARY DUTIES AND RESPONSIBILITIES: • Provides supervision of the home dialysis program; • Responsible for home dialysis program development; • Analyzes and records relevant patient health data accurately; • Analyzes assessment data with clinical team to identify patient problems; • Collaboratively (with RN team) develops a plan of care that describes interventions to attain expected outcomes; • Jointly implements and records interventions to attain expected outcomes in EMR; • Evaluates patient progress toward attainment of identified goals; • Directs and manages patient care provided by RN’s, LPN’s, PCT’s and support personnel; • Oversees the utilization of equipment and materials in a safe and cost-effective manner; • Performs and documents Point of Care testing and quality control in accordance with established P&P • Functions as patient advocate and ensures care is delivered in a non-judgmental and non-discriminatory manner; • Assures patients are informed and consents are signed for indicated procedures; • Consistently participates as an active member of department performance improvement committee; • Responsible for QAPI tracking and action plans to meet goals. Meets with the Staff RN’s monthly to drive QAPI goals; • Uses current data and research to have the home program reflect industry trends that will improve patient outcomes: • Develops relationships with colleagues at other home programs; • Provide Financial management to the program • Continually review relationships with Skilled Nursing Facilities and other facilities with an eye on contracts, quality of service & outcomes • Works collaboratively with the interdisciplinary team, other departments and outside institutions; • Works collaboratively with team members to maintain inventory; • Assesses the need for home visits; • Provides support for home patients and family members while maintaining professional boundaries; • Collaborates with MD and staff on hospitalized home patients providing direct patient care as needed • Assures clinical staff competencies • Other Duties as Assigned
Current New Jersey Professional Nurse License or eligible; BSN Preferred in this position. One full year RN experience as a Peritoneal Dialysis Nurse.
