Job Description

The out-patient Center treats chronic non healing wounds with state-of-the-art therapies. The Dept utilizes a physician-driven, multi-disciplinary approach to achieve a healing rate of 95% with a median days to heal of 31. Some of the wounds treated are: diabetic ulcers, neuropathic ulcers, pressure ulcers, venous insufficiency, traumatic wounds, surgical wounds, vasculitis, burns and other non healing wounds. Advanced modalities such as skin substitutes, sharp debridement, biologics and hyperbaric oxygen therapy are used to treat patients.

Responsibilities include:

* Coordinating care across the continuum of care setting.

* Providing education to patients, family, and nursing staff about preventive measures or techniques to optimize wound healing.

* Implementing plans to prevent pressure ulcers and other wound developments.

* Leading performance improvement projects that successfully reduce the percentage of patients whodevelop pressure ulcers and wound complications. Developing individualized rehabilitative care that is essential to facilitating the patient's return to a productive lifestyle.

* Selecting optimal stoma sites preoperatively to promote post-operative independence.

* Providing patient and family education and follow-up care.

* Identifying and addressing issues that require further attention.

Job Requirements

Current New Jersey Professional Nurse License. Ability to perform all competencies of the position is required for employment in the position. CWON Certified Wound and Ostomy Nurse certification preferred. Previous work experience in area of wound and Ostomy care.
